In Diablo 4 Season 1 (also known as the Season of the Malignant), Malignant Hearts drops from the Malignant Monsters, which are born from a deviant form of Lilith’s sinister side. Malignant Monsters are filled with rage and attack anyone in sight. But, once defeated, these creatures drop an item called Malignant Heart. Throughout the season, players can collect these items and use them exclusively on jewels instead of regular gems, and each Malignant Heart adds a category of power to the item.

Types of Malignant Hearts in Diablo 4

As per an official blog post, Diablo 4 season 1 introduces 32 Malignant Hearts in the game. Each of them provide a build-altering bonus and change your attack behavior, as you can socket the hearts in rings or medallions. Some Malignant Hearts provide an increased barrier, while others help with elemental damage build-up. In Season 1, players will come across four types of Malignant Hearts in-game, and they are listed below:

Vicious Heart: Vicious Hearts contains offensive power inside them. It drops from the Vicious malignant monsters around the world. Vicious Hearts helps in amplifying your attack damages. Brutal Heart: Brutal Hearts contains defensive power, such as barrier generation and more. Like Vicious Hearts, players can grab them by defeating brutal malignant monsters. Devious Heart: Devious Hearts provide utility power, and like every other heart in the game currently, drop from the devious malignant monsters. Wrathful Heart: These hearts are the rarest of the four in the game currently. Much like the previous types, these drops from the wrathful malignant monsters. However, the drops are very rare, and you can slot these hearts in any gear with a socket.

How to Obtain Malignant Hearts In Diablo 4

There are a few specific ways to get a Malignant Heart in Diablo 4. These involve battling malignant monsters and gathering items from them. Follow the below steps. Before doing so, make sure you have progressed through the season’s storyline:

First, track down a partially corrupted monster. Once you find one of these monsters, kill them as usual. Upon killing them, they will drop a Malignant heart.

Interact with this heart using the cage of bindings. This will revive the partially corrupted monster into a fully corrupted monster.

Take it down to cage the malignant heart and pick it up. You can now slot this Malignant Heart into your rings or amulets.

You can also craft a Malignant Heart at Cormond’s workbench. These workbenches can be found in the main towns of the game, and there is one in particular at Kyovashad. You will require Malignant Ichor to craft a caged heart in Diablo 4. Malignant Ichors, for the uninitiated, are obtainable from various chests in-game, specifically from chests received after an event. Another way of getting yourself ichor is salvaging the duplicate caged hearts at the workbench.
Once you have enough malignant ichors, you can craft a Vicious, Brutal, and Devious Malignant Heart in the game. The properties of these crafted hearts are randomized, and players cannot decide their properties. Furthermore, Wrathful Malignant Hearts drops randomly from the enemies and at higher levels and aren’t craftable.
